What Is ERP Therapy?

Exposure and Response Prevention (ERP) is the gold-standard, first-line treatment for OCD, recommended by leading organizations including the International OCD Foundation (IOCDF) and the American Psychological Association. It involves two core components:

Exposure — gradually and intentionally confronting the thoughts, images, situations, or objects that trigger your OCD.

Response Prevention — resisting the compulsions, rituals, or avoidance behaviors you would normally use to relieve distress.

Over time, ERP teaches your brain that the feared outcome either won't happen, or that you can tolerate the uncertainty without compulsing. Distress naturally decreases — not because you avoided it, but because you moved through it.

How Is ERP Different from Regular Therapy?

Most traditional talk therapy focuses on exploring feelings and gaining insight — which can be helpful for many concerns, but is not effective for OCD. In fact, some common therapy approaches (like trying to challenge or "logic away" obsessive thoughts) can make OCD worse.

ERP is specifically designed for OCD. It's structured, active, and skills-based. Sessions at Onward Healing are collaborative and goal-oriented — we work together to build a hierarchy of feared situations and systematically work through them at a pace that challenges you without overwhelming you.

What Does ERP Look Like at Onward Healing?

Treatment begins with a thorough assessment of your OCD — your obsessions, compulsions, avoidance patterns, and how OCD is affecting your daily life. From there, we work together to build an exposure hierarchy: a ranked list of situations from mildly uncomfortable to most feared.

Sessions are 50 minutes via secure telehealth video. We typically meet weekly, at least in the beginning. Between sessions, you'll have assigned exposures to practice on your own — this is where much of the real work happens. We track your progress throughout and adjust the plan as needed.

Is ERP Difficult?

Yes — and that's by design. ERP asks you to feel the anxiety without doing anything to make it go away immediately. That can be hard. But we move at a pace that's challenging without being overwhelming, and you're never pushed into anything you're not ready for.

Many clients are surprised to find that the anticipation of exposures is often worse than the exposure itself. With practice, the anxiety diminishes — not because the situation changed, but because your brain learned a new response.
